Library Card Registration Form

Standard patron Library Card Registration forms (revised 7/2019) in word format (2 sizes), that can be downloaded, customized if desired, and printed.

Driver's license number field - following PLS policies regarding security of personally identifying information, driver's license numbers will not be added to the patron's Evergreen account information and have been removed from the paper registration form. There is now an area to indicate that a patron's ID has been verified.

EAC reviewed these downloadable forms on 2/20/2024.

PLS recommends shredding paper forms as soon as the patron record has been fully entered into the Evergreen database, or, no longer than necessary for local workflow. Libraries with local policy indicating paper forms are to be kept should ensure that the forms are securely stored. ( OWWL Policy: Patrons)
